Job Title: Home Health LMSW

Experience: Minimum 1 year of skilled home health experience

License: Must have licensure in two specific states

Certifications: BCLS

Must-Have: Oasis experience required

Description: Home Health LMSW positions available in the local area. Candidates must have both state licenses. The shift is 8-hour days. The facility cannot accommodate 4x10s or 3x12s. The patient ratio is 1:1, with the potential to visit up to 6 patients per day. A productivity standard of 30 points applies. Weekend work is not required. This position does not require on-call duties. Travelers must have their own transportation. The system is efficient in grouping clients to minimize drive time. Occasional maximum drive radius is roughly 60 miles, with total drive time around 60-90 minutes per day. Candidates must have completed the primary series COVID vaccination along with one booster. Oasis experience is essential for this role. Business casual attire is expected; scrubs are allowed but not mandatory. Charting will be done using designated software on a laptop and phone app. Additional special requirements may apply.
