Meadowview Rehabilitation & Nursing Center is seeking a PRN Physical Therapist (PT) to join our dedicated in‑house rehabilitation team. This is an excellent opportunity for a therapist looking for flexible scheduling while making a meaningful impact in the lives of our residents.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services to improve mobility, strength, balance, and overall function
• Collaborate with physicians, nursing staff, therapists, and families to achieve optimal outcomes
• Complete accurate and timely documentation
• Participate in care planning and interdisciplinary team meetings
• Ensure compliance with all regulatory and facility standards

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Pennsylvania Physical Therapist license in good standing
• SNF/LTC experience preferred, but not required
• Strong communication and clinical skills
• Experience with Net Health and Point Click Care (PCC) preferred
