POSITION SUMMARY – LMSW Social Worker

POSITION DUTIES – Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

MINIMUM REQUIRED QUALIFICATIONS –

• AK LMSW
• Masters in Social Work or equal educational degree
• BLS
• 2 years social work experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break
• Experience using Epic and Midas

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F 8a-4:30p

SYSTEMS – Epic & Midas
