Job Overview

We are seeking a Licensed Practical Nurse (LPN) for a contract assignment in a long-term care and skilled nursing environment. The LPN will provide direct nursing care, administer medications, monitor patient conditions, and collaborate with the healthcare team to ensure quality patient outcomes.

Key Responsibilities

Provide direct patient care under established nursing standards

Administer medications and treatments as prescribed

Monitor, assess, and document patient conditions

Report significant changes in patient status to supervising clinicians

Maintain accurate and timely clinical documentation

Support implementation of individualized care plans

Ensure compliance with infection control and safety procedures

Promote patient comfort, safety, and well-being

Assignment Details

Start Date: 06/04/2026

End Date: 07/16/2026

Shift: Evening Shift

Shift Time: 03:00 PM 11:30 PM

Schedule: Contract Schedule (Every Other Weekend Required)

Contract Length: 7 Weeks

Guaranteed Hours: 40 Hours per Week

Requirements

Active LPN/LVN license required

Minimum 1 year of nursing experience preferred

Previous long-term care, rehabilitation, or skilled nursing experience preferred

Strong clinical assessment and documentation skills

Ability to work effectively within an interdisciplinary team

Valid state or federal photo ID required on-site at all times

Must comply with all onboarding and credentialing requirements

COVID-19 vaccination or approved exemption may be required per client policy
