WHAT YOU'LL DO
As the Care Team Manager for Rhode Island, you will be responsible for leading the daily operations of the Care Team, including Registered Nurses (RNs), Nurse Practitioners (NPs), LICSWs, Health Partners, and other staff. You will manage team members, oversee schedules, develop and enforce policies and procedures, and ensure smooth care delivery both virtually and in our patient’s homes.
As a key leader for the Rhode Island market, you will be responsible for building and developing a high performance team, establishing and meeting success metrics for the region, managing relationships with local providers and other key partners, and partnering with the Market Operations Leader and Medical Director on strategic initiatives.
Clinical Operations Leadership
Lead day-to-day clinical operations, including staffing, scheduling, and adherence to clinical SOPs
Build and sustain a high-performing, cohesive team culture that delivers outstanding patient outcomes and experience
Hire, onboard, coach, and develop clinical and administrative staff
Partner closely with the Market Operations Leader and Medical Director to ensure cohesive strategy and operations that support market growth
Data-Driven Execution
Use operational and clinical data to monitor performance, identify improvement opportunities, and drive timely action
Hold the team accountable to clear performance metrics across quality, patient experience, productivity, and operational efficiency
Foster a culture of continuous improvement, using data to spot trends, solve problems, test new approaches, and measure results
Care Delivery Oversight
Keep patient experience and outcomes are at the core of every decision
Ensure care delivery across virtual, in-person, and at-home settings is efficient and high quality
Implement strategies that improve access and quality of care while making the best use of the Care Team's time
Handle escalated patient issues and drive service recovery to maintain the highest level of patient satisfaction
Partnerships Market Development
Serve as a trusted partner to Rhode Island providers and other market stakeholders, supporting Daymark's visibility and growth through partner-facing initiatives
Cross-Functional Collaboration
Work closely with Enrollment, Clinical Operations, Tech, Marketing, and Growth teams to align Care Team efforts with organizational goal
WHAT WE ARE LOOKING FOR
5+ years of experience in healthcare operations, care management, or a related clinical leadership role, with some prior experience in a people management capacity
Strong background in patient care delivery and clinical operations. Experience with virtual care and/or at-home healthcare delivery models is a plus. Clinical background (RN, NP, LICSW, or equivalent licensure) is preferred
Experience hiring, onboarding, and developing both clinical and administrative staff in a fast-paced environment
Deep commitment to delivering exceptional quality of care and support for patients and families, with an understanding of complex care models strongly valued
Strong operational acumen, including scheduling, staffing, and budget management
Data-driven mindset with the ability to monitor clinical outcomes, track team performance metrics, and use data to drive continuous improvement
Excellent interpersonal and communication skills, with the ability to build trust with clinical staff, community partners, and patients and families alike
Comfort with ambiguity and startup environments; ability to build and iterate on processes while maintaining high standards of care delivery
Additional Information
The expected compensation for this role is $120,000 - $150,000 annually.
This role will be in person Monday-Friday from 8:30 AM to 5 PM ET in our Providence, RI office.
