We are urgently seeking a dedicated and compassionate Full-Time Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses to treatment plans
- Administer medications via injections, IV infusions, and other routes while adhering to strict safety protocols and medication administration guidelines
- Manage wound care, catheterization procedures, tube feeding, and sterile processing techniques to promote optimal healing and comfort
- Document all patient interactions accurately using Electronic Health Records (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies
- Assist with diagnostic evaluations including specimen collection/processing and supporting medical imaging procedures when necessary
- Provide specialized care for geriatric patients, including dementia and memory care, Alzheimer’s care, and behavioral health support
- Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care regulations to ensure seamless patient transitions across settings
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction
- Proven experience in diverse clinical settings such as long-term care facilities, hospitals (including ICU or trauma centers), emergency rooms, or home & community care environments
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and infection control practices
- Experience working with individuals with disabilities or developmental delays is highly desirable
- Ability to perform physical examinations and assessments efficiently while demonstrating excellent patient care skills
- Certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) preferred but not required; willingness to obtain if necessary
- Excellent communication skills coupled with a compassionate approach to patient-centered care

Pay: $30.00 - $35.00 per hour
