Liquid Personnel is seeking a dedicated Social Worker to join its client's Hospital Discharge Team in the London Borough of Richmond upon Thames. In this role, you will play a crucial part in ensuring safe, timely, and person-centred hospital discharges, helping adults return to their homes or community settings with the right care and support. This position promotes independence, wellbeing, and embraces a “Home First” approach.
What will your responsibilities be?
In this role, you will:
Coordinate and manage the discharge process for adults with diverse and complex needs.
Work within the Discharge to Assess (D2A) framework to facilitate timely transitions.
Undertake Care Act 2014-compliant assessments, including Mental Capacity Act assessments and Best Interest decisions.
Develop strengths-based, person-centred care plans that promote independence and choice.
Collaborate with health professionals, therapy teams, housing providers, and voluntary sector partners.
Identify and respond to safeguarding concerns in line with statutory guidance and local policy.
Maintain accurate records and ensure adherence to relevant legislation (Care Act, MCA, DoLS).

Qualifications Experience:
To be successful in this role you must have:
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
