Speech-Language Pathologist SLP – Skilled Nursing Facility

Weare seeking a skilled and compassionate Speech-LanguagePathologist to join a Skilled Nursing Facility inModesto, California.This role focuses on working with an adult and geriatricpopulation, helping patients recover from stroke, neurologicalconditions, and other medical issues affecting communicationand swallowing. You will play a key role in improving patients’quality of life by delivering individualized therapy plans thatsupport functional communication and safe swallowing. Job Responsibilities

  • Evaluate, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders in patients
  • Develop and implement individualized treatment plans based on patient needsand clinical findings
  • Provide therapy focused on speech, cognition, and dysphagia management
  • Monitor patientprogress and adjust treatment plans as needed
  • Educate patients, families, and caregivers on therapy techniques and safety strategies
  • Maintain accurate and timely documentation in compliance with facility and regulatory standards

Requirements

  • Active state license as a Speech-Language Pathologist in California
  • Current BLS/CPR certification
  • Minimum of 6months of recent SLP experience within the last 5 years preferred
  • Experience in a Skilled Nursing Facility or geriatric setting is a plus
  • Strong clinical and communication skills
