Licensed Practical Nurse Hospice

Job Description Medi Home Health & Hospice , a proud member of the Medical Services of America family, is seeking an experienced Full-Time Licensed Practical Nurse (LPN) for our Hospice patients in Winston Salem (Thomasville/Davidson) NC. As a member of the multidisciplinary team, the LPN is responsible for performing selected nursing care under the direction of a registered nurse. Responsibilities Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care in accordance with agency policies. Adherence to policies and procedure. Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals. Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy. Some on call rotation. Job Requirements Successful completion of a certified licensed practical nursing program. Currently licensed as an LPN in the state of NC. Minimum of one year of clinical experience. Hospice experience strongly preferred. Reliable transportation and company required auto liability insurance.
