Facility 4: The Grand at Utica – Utica, NY

Facility 5: The Grand at Maple City – Hornell, NY

Facility 6: The Grand at Rome – Rome, NY

Schedule: Full-Time

Salary per annum: $150,000 - $200,000

Job Summary

We are seeking a friendly and compassionate Endocrinology Nurse Practitioner/Physician Assistant to join our team. As an Endocrinology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals.

Responsibilities

Assist care coordinators across the different providers and settings

Promote better health outcomes through communication between facilities and patients

Assess patients with Endo conditions and report through our proprietary software system

You will be cross-trained in other areas of care management

Work under the supervision of an off-site Collaborative Physician

Gathering and analyzing patient information

Detailed review of the system

Review of all pertinent patient labs, studies, and provider notes

Requirements

Minimum 6 months of experience in long-term care or a skilled nursing facility

Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license

DEA License

CPR BCLS/BLS certified

Must hold a Master's Degree in Nursing

Completed a degree from an accredited Nurse Practitioner program

A year of experience in Endocrinology

Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided

Must have great organizational and time management skills.

Reliable transportation

Must be willing to travel to different nursing homes within the state of Florida (100% commitment)
