Role Scope : Senior Director of Growth Operations
The Senior Director of Growth Operations scales and oversees day-to-day participant growth execution across our centers, reporting to the SVP of Growth. This person will lead Outreach and Enrollment teams directly, partnering with Center Executive Directors, Medical Directors, Clinical Pre-Enrollment, and central Growth to consistently hit and exceed growth targets. Responsibilities include coaching front-line supervisors and staff, optimizing workflows, improving enrollment performance, and building scalable, mission-aligned growth systems — all critical to expanding access to PACE with integrity.
The ideal candidate has strong operational leadership experience, thrives in fast-paced settings, and is energized by building steady-state systems, coaching high-performing teams, and driving mission-driven growth.
Core Responsibilities Expectations for the Role
Serve as the senior-most field-facing growth leader, accountable for participant growth targets and performance across multiple centers.
Directly manage and develop current Outreach and Enrollment Supervisors (2) and subsequent supervisors for each new center that is launched.
Oversee all non-clinical Outreach and Enrollment staff, ~6-7 per center including Outreach and Enrollment Supervisors, across Habitat Health centers. Includes setting and managing performance expectations, providing ongoing coaching and development, and building a unified, mission-aligned growth team culture.
Rapidly scale and grow the Outreach and Enrollment team by standing up new Outreach and Enrollment teams as Habitat Health opens new centers
Manage and develop Ops Quality and Excellence department to capture learnings across centers, provide resources, skill training, and maintain accountability for successful community outreach, engagement of new prospects, and enrollment opportunities.
Implement sustainable growth operations at the center level, including referral engagement, community outreach, prospective participant case management, and participant handoffs to the PACE center interdisciplinary team.
Work across the Central Growth Team to develop and implement new programmatic strategies to improve the performance and processes of the Outreach and Enrollment teams
Work cross-functionally with clinical enrollment teams, including the Pre-Enrollment Clinical Team to assess PACE eligibility and submit DHCS level of care applications for prospective participants.
Collaborate with Central Growth Team to manage and develop coordination and handoffs with centrally run growth programs and large-scale partners (e.g., Kaiser Permanente)
Regularly review team performance data, identify gaps or bottlenecks, and implement solutions in partnership with local and central teams.
Partner closely with the Central Growth Team to develop the playbook for growth-focused operational execution at Habitat—developing systems and standards that can scale as we expand to new markets.
Partner closely with Center Executive Directors and Medical Directors to align outreach and enrollment activities with clinical and operational capacity.
Represent the Outreach and Enrollment team in broader growth discussions, ensuring central strategy incorporates learnings from the centers and that teams are equipped to execute organizational priorities.
Contribute to the future structure of the growth function and local Outreach and Enrollment Teams, including identifying internal leaders, shaping team design, and evolving operating models.
Ensure an exceptional experience for all prospective participants, in alignment with Habitat’s values.

Required Qualifications:
7-10+ years of experience in healthcare, field growth, sales, consulting, or service-based leadership roles.
Experience making strategic decisions based on P L performance, with a clear impact on business outcomes.
