FULL-TIME / SALARY POSITION
Elite Care Management is seeking an experienced Home Health Clinical Manager (RN) to support our Indiana office. This role includes both office-based clinical leadership and field responsibilities to ensure high-quality, compliant patient care.

Elite Care Management is a privately owned home health care company with over 26 years of experience in providing compassionate and personalized care to patients in their homes.
Responsibilities:
Provide clinical oversight and case management for home health patients
Review OASIS, plans of care, and clinical documentation
Supervise and support field clinicians (RNs, LPNs, therapists, CNAs/HHAs)
Perform supervisory and patient field visits as needed with rotating on-call
Ensure compliance with Medicare, Indiana State Regulations, and accreditation standards
Participate in QAPI, infection control, and quality improvement activities
