Traveler Weekly Blended Pay : $2,600 weekly ($20/hr w2 + $1,800 week stipend)

Position Summary:

We are seeking a compassionate and dedicated Master of Social Work (MSW) to provide comprehensive psychosocial support and care coordination for patients and their families. The ideal candidate will assess patients' social, emotional, and environmental needs, develop care plans, connect individuals with community resources, and collaborate with interdisciplinary healthcare teams to improve patient outcomes.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments.
• Develop and implement individualized care and discharge plans.
• Provide crisis intervention, counseling, and emotional support to patients and families.
• Coordinate with physicians, nurses, case managers, and community agencies to ensure continuity of care.
• Connect patients with community resources, financial assistance, housing, transportation, and support services.
• Advocate for patients' rights and ensure culturally competent, patient-centered care.
• Participate in multidisciplinary care team meetings and case conferences.
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Educate patients and families regarding available community programs and healthcare resources.
• Ensure compliance with hospital policies, regulatory requirements, and ethical standards.

Experience Requirements:

• Years of Experience: 2 year(s)
• Years of experience 1 full year of acute care experience is required to redirect if they do not have.
• Must be recent hospital experience, within the last 3 months.
• BLS, LCSW*, MSW
