Case Manager III

Manage 30+ clients with complex needs, including disabilities, long-term homelessness, or institutional histories

Coordinate with hospitals, detox units, and behavioral health providers

Lead SSVF’s shallow subsidy and healthcare navigation efforts

Train and support junior case management staff

Conduct outreach in rural and extreme high-risk areas (e.g., Wetlands Park, Laughlin, Logandale)

Provide direct case management services to homeless and at-risk veterans and their families.

Screen veterans’ eligibility for the program and facilitate the intake process.
