Hospice Social Worker

Social Worker | MSW, LCSW

Location: Boise, ID

Pay: $32-37 /hour

Hours: Full-time

Job Overview:

We are seeking a compassionate and dedicated social worker to provide support to the geriatric community. The ideal candidate will have a strong background in providing end of life support, offering community resources, and discussing care goals with patients. We are also looking for someone to help coordinate the volunteer list for hospice.

Responsibilities:

• Communicate effectively with the internal team and those offering patient support
• Organize schedule in an efficient manner to travel to assigned visits
• Provide helpful resources and discuss care goals with elderly patients
• Navigate end of life issues that require support and attention
• Offer crisis management support during difficult situations
• Coordinate the volunteer list for hospice patients

Qualifications:

• One of the following licensures MSW, LMSW, LCSW
• Valid driver's license
• Experience with geriatric community preferred, not required
