SUMMARY

  • Position Type: Hourly role, available as full-time or part-time.
  • Scope of Role: This role involves providing direct support within a single program or unit, typically assisting 4 - 6 individuals in residential settings or up to 10 in periodic programs.
  • Key Responsibilities:• Deliver support as outlined in individual service plans, including socialization, health maintenance, medication administration, and skill development.
  • Assist with daily activities such as personal care, housekeeping, recreational activities, transportation, community integration, shopping, and more.

ESSENTIAL JOB FUNCTIONS

To succeed in this role, one must be able to effectively carry out each essential function as outlined below:

Services and Supports

  • Direct Support: Provide direct assistance as per individual plans; aid in socialization, behavior development, personal care, and community orientation.
  • Records: Maintain accurate documentation of progress; review logs for updates in service plans; uphold confidentiality.
  • Management of Individuals’ Assets: Make purchases within the individual's budget, aligning with their preferences.
  • Relationships: Maintain positive, professional relationships with clients, their friends, families, and case managers; adhere to customer service standards.
  • Reporting: Report any suspected abuse or neglect; document medical, behavioral, and other incidents in line with policy.
  • Rights: Maintain confidentiality, respect client rights, adhere to universal precautions, and support individuals in exercising their rights.

Health Care

  • Appointments: Accompany clients to medical appointments if needed; communicate provider instructions effectively.
  • Medications: Manage medication administration or support self-administration accurately; report any errors promptly.
  • General Health Care: Monitor and document health concerns; call emergency services if needed; comply with healthcare directives.
  • Medical Supplies and Equipment: Follow protocols for medical equipment use; report any malfunctions.
  • Meal Preparation: Assist in meal preparation according to planned menus and dietary needs; ensure correct implementation of dining plans.

Employment Responsibilities

  • Training: Attend all required orientation and ongoing training sessions; participate in staff meetings.
  • Employee Scheduling: Maintain punctuality; inform supervisor when unable to work as scheduled; fill in shifts when necessary.
  • Teamwork: Collaborate with others, communicate openly, and act courteously and respectably.
  • Workplace Safety: Contribute to maintaining a safe work environment; report hazards and participate in safety training.

Maintenance

  • Vehicles: Transport individuals safely; use safety equipment appropriately; report all accidents and concerns.
  • Housekeeping: Perform housekeeping tasks to maintain a neat and safe environment; report hazards.
  • Maintenance and Repair: Perform minor repairs; ensure environmental safety; report significant issues to the supervisor.
  • Safety: Monitor water temperatures for safety during bathing and participate in safety drills.

Other

  • Fulfill additional duties as needed.
  • Utilize Mobile Punch for timekeeping.

Minimum Knowledge and Skills Required

The following are the qualifications and skills needed for this role:

Education and Experience

  • High school diploma or equivalent preferred
  • Six months of experience in human services preferred
  • Minimum age of 18 years
  • Basic computer knowledge

Certificates, Licenses, and Registrations

  • Valid driver's license in good standing
  • Vehicle registration and insurance if used for client transportation
  • Completion of all required state training and certification
  • Maintain a good driving record; adherence to company vehicle policy required

Work Environment

  • Work primarily takes place at the program site with occasional community outings.

Physical Requirements

  • Heavy Work: Lifting up to 100 pounds occasionally, 50 pounds frequently, and 20 pounds consistently for moving items.
