Job Purpose:

  • The San Antonio LPN at SOLUTIONS HEALTHCARE is responsible for providing high-quality patient care under the supervision of registered nurses and physicians. This role is vital in ensuring that patients receive compassionate and efficient healthcare services, contributing to their overall well-being and recovery.

Key Responsibilities:

  • Administer medications and injections as prescribed by healthcare providers.
  • Monitor patient vital signs and report any changes or concerns to the medical team.
  • Assist with patient hygiene, mobility, and daily living activities.
  • Maintain accurate and timely patient records and documentation.
  • Provide emotional support and education to patients and their families regarding medical conditions and treatments.
  • Collaborate with the healthcare team to develop and implement patient care plans.
  • Ensure compliance with healthcare regulations and standards.
  • Participate in ongoing training and development to enhance nursing skills and knowledge.

Qualifications

Required Education:

  • Completion of an accredited Licensed Practical Nurse (LPN) program.
  • Current and valid LPN license in the state of Texas.

Required Experience:

  • Minimum of 1-2 years of clinical experience in a healthcare setting.
  • Experience working in a team-oriented environment, preferably in a long-term care or rehabilitation facility.
  • Proven track record of providing high-quality patient care and maintaining patient confidentiality.

Required Skills and Abilities:

  • Strong understanding of medical terminology and healthcare practices.
  • Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare team members.
  • Ability to assess patient conditions and implement appropriate care plans.
  • Proficiency in using electronic health records (EHR) systems.
  • Strong organizational skills and the ability to manage multiple tasks efficiently.
  • Compassionate and empathetic approach to patient care.
  • Ability to work flexible hours, including evenings and weekends, as needed.
