Job Purpose:
- The San Antonio LPN at SOLUTIONS HEALTHCARE is responsible for providing high-quality patient care under the supervision of registered nurses and physicians. This role is vital in ensuring that patients receive compassionate and efficient healthcare services, contributing to their overall well-being and recovery.
Key Responsibilities:
- Administer medications and injections as prescribed by healthcare providers.
- Monitor patient vital signs and report any changes or concerns to the medical team.
- Assist with patient hygiene, mobility, and daily living activities.
- Maintain accurate and timely patient records and documentation.
- Provide emotional support and education to patients and their families regarding medical conditions and treatments.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Ensure compliance with healthcare regulations and standards.
- Participate in ongoing training and development to enhance nursing skills and knowledge.
Qualifications
Required Education:
- Completion of an accredited Licensed Practical Nurse (LPN) program.
- Current and valid LPN license in the state of Texas.
Required Experience:
- Minimum of 1-2 years of clinical experience in a healthcare setting.
- Experience working in a team-oriented environment, preferably in a long-term care or rehabilitation facility.
- Proven track record of providing high-quality patient care and maintaining patient confidentiality.
Required Skills and Abilities:
- Strong understanding of medical terminology and healthcare practices.
- Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare team members.
- Ability to assess patient conditions and implement appropriate care plans.
- Proficiency in using electronic health records (EHR) systems.
- Strong organizational skills and the ability to manage multiple tasks efficiently.
- Compassionate and empathetic approach to patient care.
- Ability to work flexible hours, including evenings and weekends, as needed.
