Job Summary

The Licensed Vocational Nurse (LVN) provides compassionate, high-quality nursing care under the direction of a Registered Nurse (RN), physician, or other licensed healthcare provider. The LVN is responsible for delivering direct patient care, monitoring patient conditions, administering medications, documenting care, and supporting the healthcare team to ensure safe and effective patient outcomes.

Essential Duties and Responsibilities

Provide direct nursing care to patients according to established care plans and physician/provider orders.

Administer medications and treatments safely while monitoring patient responses.

Monitor and document vital signs, symptoms, and changes in patient condition.

Assist physicians, Registered Nurses, and other healthcare providers with patient care and procedures.

Maintain accurate and timely documentation in the electronic medical record (EMR/EHR).

Collect specimens and perform assigned nursing procedures.

Educate patients and families regarding medications, treatments, care plans, and health maintenance.

Follow infection prevention and control procedures and maintain patient safety standards.

Recognize changes in patient condition and promptly report concerns to the supervising nurse or provider.

Maintain patient confidentiality and comply with HIPAA regulations.

Collaborate with the interdisciplinary healthcare team to provide coordinated patient care.

Maintain a clean, safe, and organized care environment.

Participate in staff meetings, training, and quality improvement activities.

Perform additional duties as assigned.
