Licensed Practical Nurse - Home Care

Responsibilities:

• Complete client reassessments.
• Perform skilled nursing visits in patients' homes.
• Monitor and document patient conditions.
• Communicate changes in condition with the RN Supervisor and care team.
• Maintain accurate and timely documentation.
• Provide compassionate, professional care while following agency policies and state regulations.
• Qualifications:

• Home health or home care experience preferred but not required.
