Location: Community-Based Across Southwestern Ohio (Blue Ash training required)
Job Type: PRN (Paid per assessment)
Hours: Daytime only (Monday–Friday)

At COA, PRN Licensed Social Workers and Registered Nurses conduct person-centered assessments that help older adults remain independent. You’ll make a meaningful impact while enjoying flexibility, autonomy, and the satisfaction of mission-driven work.
What You’ll Do
- Conduct telephonic and in-person assessments to determine program eligibility
- Coordinate wraparound services for individuals with complex medical, behavioral, or social needs
- Educate clients and caregivers on available resources and care planning
- Advocate for clients facing health and safety risks or potential institutionalization
- Collaborate with internal teams, healthcare providers, and community partners
What We’re Looking For
- Active Ohio LSW or RN license in good standing
- At least one year of experience in medical social work, home health, or geriatrics
- Strong understanding of aging-related issues and community resources
- High-speed home internet for accessing agency systems
- Valid driver’s license and reliable transportation
- Excellent communication, empathy, and clinical judgment
- Above-average technology skills and adaptability
