Position Summary

The Social Worker Case Manager works closely with an RN as part of an interdisciplinary team to provide supportive, client-centered HIV/AIDS case management services through the AIDS Medi-Cal Waiver Program. This position serves a diverse client population in their home setting and focuses on helping clients remain stable, connected to care, and supported in the community.

Responsibilities

  • Partner with an RN to provide supportive, client-centered HIV/AIDS case management services in the home setting
  • Conduct biannual assessments and maintain monthly follow-up with clients between home visits
  • Assist clients with accessing and remaining connected to appropriate medical, mental health, AOD, and community-based services
  • Help stabilize clients’ health and social needs while working to avoid unnecessary higher levels of care
  • Maintain accurate and timely documentation of client interactions, assessments, and care coordination activities
  • Build trusting relationships with clients while providing compassionate and culturally sensitive support
  • Collaborate with community providers and internal team members to ensure continuity of care

Education/Training

  • Minimum of a Bachelor’s Degree in Social Work, Counseling, Psychology, or a related field
  • Equivalent degree qualifications may be considered

Experience

  • Minimum of 2 years of experience providing case management or related services
  • Experience working with HIV/AIDS clients or similar populations is preferred
  • Experience providing supportive services in home or community-based settings is preferred

Preferred Qualifications

  • Bilingual in English and Spanish preferred
  • Strong communication and interpersonal skills
  • Ability to work effectively with diverse populations
  • Compassionate, organized, and able to work both independently and as part of a team

Job Type: Full-time

Pay: $28.00 - $30.00 per hour
