The Housing Case Manager supports the Day 1 Family Fund’s mission to stabilize families experiencing homelessness or housing insecurity by providing comprehensive, intensive case management, housing navigation, and coordinated service delivery. This role is responsible for conducting thorough assessments of family needs, developing individualized housing stabilization plans, and guiding households through the process of securing safe, stable, and sustainable housing.
The Housing Case Manager works collaboratively with families to establish trust, identify and address barriers to housing stability, and connect clients to critical resources, including emergency assistance, transitional housing, and long-term housing solutions.
