RN / LPN Home Health Care 1:1 immediate start 6:30 am -3:30 pm $25-$30

Job Summary

We are seeking a compassionate and dedicated RN or LPN for our Home Health team. The ideal candidate will provide high-quality patient care in a home setting, ensuring the comfort and well-being of patients while promoting their independence. This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with patients, families, and healthcare teams.

Duties

• Conduct comprehensive patient assessments to develop individualized care plans.
• Administer medications and treatments as prescribed, and wound care.
• Perform nursing procedures and vital sign monitoring.
• Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
• Educate patients and families on health management, medication administration, and home care techniques.
• Document patient progress and maintain accurate medical records in compliance with HIPAA regulations.
• Provide support for patients with chronic conditions, including pain management and critical care needs.

• Engage in ongoing professional development to stay current with best practices in home health nursing.

Requirements

• Valid Registered Nurse (RN) license or LPN in the state of practice.
• Experience in home health care or similar settings is preferred.
• Proficiency in medical terminology, patient assessment, and basic life support techniques.
• Previous experience working with diverse populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus.
• Excellent interpersonal skills to effectively communicate with patients, families, and healthcare professionals.
