In-Home Health Nurse Practitioner or Physician Assistant

Signify Health, part of CVS Health, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships for better health outcomes. Clinicians will conduct comprehensive health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests as ordered by health plans.

Practice Info

• Primary workplace is the private residence of health plan members
• Visits last about 45 minutes
• Use an iPad and an intuitive clinical workflow for each evaluation
• Not required to prescribe medicine, order lab tests, or alter current treatment regimens

Responsibilities

• Provide In-Home Health Evaluations
• Engage with people in the comfort of their own homes
• Conduct medication and medical history reviews
• Perform physical evaluations
• Conduct additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.) if ordered by the person's health plan
• Travel locally and within licensed states, as needed
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products

Compensation

• The typical pay range for this role is: $46.03 - $99.14
