Description
- Conduct thorough evaluations of patients in various settings, whether inpatient or outpatient, crafting customized treatment plans that reflect individual needs.
- Deliver targeted physical therapy interventions that promote mobility, enhance strength, and improve balance and overall function.
- Work alongside nurses, physicians, case managers, and fellow rehabilitation professionals to ensure optimal patient outcomes.
- Guide patients and their families through therapeutic goals, discharge planning, and exercises tailored for home practice.
- Maintain detailed documentation of evaluations, treatments, and patient progress following facility and regulatory standards.
- Engage in care team rounds and interdisciplinary meetings to facilitate coherent and effective patient care.
Education
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
Certifications
- Valid state licensure as a Physical Therapist
- CPR certification preferred
- Specialized certifications such as neurological or geriatric are advantageous but not mandatory.
Skills
- Prior experience is highly desirable.
- Capability to manage patients with intricate medical and mobility challenges.
- Excellent communication and collaboration skills.
- Familiarity with digital charting and clinical documentation systems is expected.
