Case Manager - Adult Bridge

$58K/year Salary! Full benefit package and up to 34 Paid days off 1st year! Bridge Teams provide intensive, community services to individuals with severe mental illness. Clients are transitioning from recent psychiatric hospital discharge to outpatient services and often have complex mental health needs, including acute psychosis, trauma, and co-occurring substance use disorders. The primary goal of Bridge Teams is to help reduce hospitalizations and incarcerations, by providing the resources essential to thrive and maintain independence. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
