Position Overview
Pay range: $55,180 - $89,670 / year
Sign-on bonus: $7,500 sign-on bonus may be available

Job Description
To coordinate, negotiate and obtain services and resources for the care of patients across the care continuum. To provide interventions designed to promote health, prevent disease, and address barriers of access to healthcare. To facilitate the discharge and or transitions planning process and identify the most appropriate community resources to assist patients and their caregivers. To evaluate medical necessity and cost effective methods of patient care while promoting value for the patient and the organization.
Qualifications
Bachelor's degree in Nursing and three (3) years of related experience required OR Associate's degree in Nursing and five (5) years of related experience required OR Master s Degree in Social Work required and 2 years experience in a healthcare care setting preferred. Work experience may NOT substitute for education requirement. Currently licensed as either a Registerd Nurse (RN) by the Alabama Board of Nursing or eligible Nursing Licensing Compact or Licensed Master's Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required. Nurses require certification in Basic Life Support (BLS) by the American Heart Association (AHA) or should be obtained with in 45 days of employment. Nurses may be required to obtain Advanced Cardiac Life Support (ACLS) certificaiton and/or Pediatric Advanced Life Support (PALS) from the American Heart Association. LMSWs will be required to complete the First FIve Healthstream within 45 days of employment.Why Work at UAB Medicine
