Schedule / Job Type

Contract | Days | Monday through Friday with one weekend day on-call rotation | 5x8-Hour Shifts (8:00 AM – 5:00 PM)

Key Responsibilities

• Provide occupational therapy services to patients in a home health setting
• Complete a minimum of 25 patient visits per week to fulfill guaranteed hours requirement
• Complete timely documentation in accordance with home health standards, including OASIS assessments
• Manage an assigned patient territory with personal transportation between visits
• Participate in on-call rotation as required
• Complete required orientation modules during onboarding

Required Qualifications

• Active California Occupational Therapist license (candidates with pending CA license who have applied at time of submission will be considered)
• BLS certification through the American Heart Association (AHA) required
• Active, valid driver's license and personal transportation required
• OASIS experience required
• Timely communication and documentation are mandatory
• Completion of Home Health Checklist is mandatory

Preferred Experience / Skills

• Minimum of 1 year of home health experience preferred; candidates without home health experience will be considered
• Familiarity with home health compliance and documentation standards
