Hybrid Caregiver (Day Center & Homecare)

Care Resources is a Program of All-Inclusive Care for the Elderly (PACE), serving adults 55+ with a focus on helping participants maintain independence and avoid nursing home placement. Our interdisciplinary team delivers high-quality, holistic care that supports physical, mental, and social well-being.

We are seeking a Hybrid Caregiver who will provide care both in our Day Center and in participants’ homes.

Schedule

  • Full-time, Monday–Friday
  • 32–36 hours per week

Position Summary

The Hybrid Caregiver supports participants across two care environments:

In the Day Center

  • Assist with personal care (bathing, grooming, toileting, feeding)
  • Support mobility and use of assistive devices
  • Escort participants to activities and appointments
  • Monitor and report changes in condition
  • Assist with recreational and physical activities
  • Support safe entry/exit from transportation

In the Kitchen (as needed)

  • Prepare and serve meals based on dietary needs
  • Clean and sanitize kitchen and dish areas
  • Stock and manage pantry inventory
  • Follow food safety standards and complete logs
  • Provide personal care (bathing, dressing, grooming)
  • Assist with meal preparation and light housekeeping
  • Support daily living tasks (laundry, errands, shopping)
  • Provide companionship and recreational support
  • Ensure a safe and clean home environment

Requirements

  • Valid driver’s license, reliable vehicle, and auto insurance
  • Ability to read, write, and communicate in EnglishAbility to stand for extended periods and lift up to 60 lbs
  • Pass basic physical and background check

Pay

  • Non-Certified: Starting at $20.11/hour
  • Certified: Starting at $21.36/hour
  • Additional $2.00/hour incentive for all hours worked in the homecare setting
