Case Manager - Assertive Community Treatment

$58K+/year Salary plus overflowing benefits and PTO! Case Manager is part of a team and responsible for individual, family and/or group services that assists individuals in sustaining recovery. Case Managers working on ACT teams is responsible for targeted case management services, including advocacy, linkage, referral, and resource acquisition. Services are predominantly delivered to an adult population with a serious mental illness, substance abuse, co-occurring and/or medically co-morbid diagnosis. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
