Kahu Malama Nurses is seeking a Physical Therapist to join a growing and dynamic team!
Job Description
- Perform comprehensive physical therapy evaluations, re-evaluations, discharge assessments, and daily treatment sessions, and develop individualized treatment plans.
- Assess patients' functional limitations, strength, mobility, balance, and pain levels.
- Implement evidence-based therapeutic interventions, including therapeutic exercise, manual therapy, neuromuscular re-education, and functional training.
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
- Educate patients and family members regarding treatment plans, home exercise programs, injury prevention, and wellness strategies.
- Maintain accurate and timely documentation in accordance with clinic policies, regulatory requirements, and payer guidelines.
- Collaborate with physicians, healthcare providers, and support staff to coordinate patient care.
- Ensure compliance with state licensure requirements, professional standards, and clinic protocols.
- Participate in quality improvement initiatives and continuing education activities.
- Maintain a safe, clean, and professional treatment environment.
Experience And Education Requirements
- Doctor of Physical Therapy (DPT) or Master's Degree in Physical Therapy from an accredited physical therapy program.
- Credentialed with major insurance carriers, including HMSA, TRICARE, Medicare, UHA, MDX, HMAA, and HMA.
- Current Physical Therapist license in good standing within the state of practice.
- CPR/BLS certification required.
- Strong knowledge of orthopedic, neurological, and musculoskeletal rehabilitation.
- Excellent interpersonal, communication, and patient education skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency with electronic medical records (EMR) systems preferred.
Preferred Qualifications
- Experience in an outpatient orthopedic or sports medicine setting.
- Certification in specialty areas such as Orthopedic Clinical Specialist (OCS), Manual Therapy, or Sports Rehabilitation is preferred but not required.
- Experience treating a diverse patient population across various age groups.
