Position Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team on the overnight shift (11:00 PM – 7:00 AM). In this role, you will provide high-quality care to residents, ensure their safety and comfort overnight, and support individualized care plans.

Key Responsibilities

  • Provide direct care to residents in accordance with individualized care plans
  • Administer medications in compliance with state regulations, company policies, and best practices (including proper documentation and follow-up)
  • Perform resident assessments as required by state regulations and community policy
  • Monitor residents for any changes in condition and promptly report concerns to the supervisor or Director of Assisted Living
  • Accurately document care, observations, communications, and clinical procedures before the end of each shift
  • Assist in maintaining and updating individualized care plans
  • Support a safe, calm, and comfortable overnight environment for residents
  • Perform additional duties as assigned

Supervision & Reporting

This position reports to the Director of Assisted Living and/or Director of Memory Care. While working independently during overnight hours, you will be expected to use sound clinical judgment and may assist in guiding or supporting other team members as needed.

Qualifications

  • High school diploma or GED required
  • Active Licensed Practical Nurse (LPN) license in the state required
  • Current CPR certification required
  • Minimum of 1 year of related nursing experience preferred
  • Strong clinical judgment, communication, and documentation skills

Physical Requirements

  • Ability to sit, stand, walk, climb stairs, bend, stoop, kneel, and reach throughout the shift
  • Ability to lift up to 25 pounds unassisted
  • Ability to assist with transferring, repositioning, pushing, and pulling residents (with assistance or equipment as needed)
