Empower patients while advancing your nursing career as a Home Health LPN in a rewarding field-based contract role. This position is perfect for skilled professionals with a strong home health background who thrive in an autonomous environment, providing compassionate care within patients homes and promoting independence and recovery.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility based on patient needs
  • Manage a caseload of approximately 2532 points weekly (about 35 patient visits per day)

Key Responsibilities:

  • Perform thorough patient assessments and implement individualized care plans.
  • Administer medications, wound care, IV therapy, and other clinical interventions as instructed.
  • Monitor and promptly communicate patient condition changes to RN Case Managers or physicians.
  • Provide education to patients and caregivers regarding disease processes, medication regimens, and self-care skills.
  • Accurately document care and communication using Home Care Home Base (HCHB).
  • Collaborate closely with the interdisciplinary team for effective and compliant patient care.
  • Adhere to infection control protocols and maintain the highest patient safety standards.
  • Participate in case conferences, ongoing education, and quality initiatives.

Required Qualifications:

  • Active, unrestricted LPN license in the state of practice.
  • Minimum of 1 year of recent home health, hospice, or related community-based experience.
  • Proven proficiency with Home Care Home Base (HCHB).
  • Valid drivers license, reliable insured transportation.
  • Current BLS certification.
  • Excellent clinical assessment, time management, and communication skills.
  • Ability to work independently in a dynamic home health setting.

Preferred:

  • Experience with Medicare-certified home health agencies.
  • Excellent understanding of home health compliance standards.
  • Strong patient-centered interpersonal skills.
