Empower patients while advancing your nursing career as a Home Health LPN in a rewarding field-based contract role. Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
- Manage a caseload of approximately 2532 points weekly (about 35 patient visits per day)
Key Responsibilities:
- Perform thorough patient assessments and implement individualized care plans.
- Administer medications, wound care, IV therapy, and other clinical interventions as instructed.
- Monitor and promptly communicate patient condition changes to RN Case Managers or physicians.
- Provide education to patients and caregivers regarding disease processes, medication regimens, and self-care skills.
- Accurately document care and communication using Home Care Home Base (HCHB).
- Collaborate closely with the interdisciplinary team for effective and compliant patient care.
- Adhere to infection control protocols and maintain the highest patient safety standards.
- Participate in case conferences, ongoing education, and quality initiatives.
Required Qualifications:
- Active, unrestricted LPN license in the state of practice.
- Minimum of 1 year of recent home health, hospice, or related community-based experience.
- Proven proficiency with Home Care Home Base (HCHB).
- Valid drivers license, reliable insured transportation.
- Current BLS certification.
- Excellent clinical assessment, time management, and communication skills.
- Ability to work independently in a dynamic home health setting.
Preferred:
- Experience with Medicare-certified home health agencies.
- Excellent understanding of home health compliance standards.
- Strong patient-centered interpersonal skills.
