Position Summary
The Inpatient Care Manager - MSW utilizes their expertise to ensure comprehensive care for a specific patient population, spanning the entire continuum of care. This position includes assessing, planning, executing, and evaluating personalized care plans, while facilitating safe and timely discharge processes. The Inpatient Care Manager - MSW functions as a clinician, care manager, and educator, collaborating effectively with interdisciplinary teams to achieve superior, cost-effective patient outcomes. Furthermore, this role involves actively managing resources to reduce unnecessary healthcare utilization and supporting performance improvement initiatives to enhance patient care and operational efficiency.
Key Responsibilities
- Conduct thorough assessments upon patient entry to evaluate anticipated length of stay, discharge requirements, and resource needs, while developing personalized care and transition plans alongside a multidisciplinary team.
- Facilitate safe and timely discharge planning, which includes obtaining post-acute authorizations, organizing transportation, home care, and facility placements, and coordinating transfers as necessary.
- Monitor and oversee patient progression through daily reviews, participation in multidisciplinary rounds, and proactively identifying and addressing barriers to discharge.
- Advocate for patients and their families by addressing clinical, educational, and psychosocial needs, ensuring their preferences are integrated throughout the care process.
- Collaborate with multidisciplinary healthcare teams while engaging patients and families in creating and implementing care plans.
- Maintain up-to-date knowledge regarding Medicare, Medicaid, and key payer benefits and reimbursement practices.
- Assist patients and families with self-management through education and support, including visits and telephonic engagement, while encouraging adherence to care plans.
- Communicate effectively with physicians, patients, families, and healthcare team members to facilitate transition planning.
- Interact professionally with colleagues, visitors, and other staff, upholding the values of the organization.
Minimum Qualifications
- Master’s Degree in Social Work.
- Experience from a relevant MSW internship program (less than 1 year accepted).
- Ability to engage effectively with individuals from diverse backgrounds.
- Strong organizational skills to efficiently coordinate care plans.
- Proficient in computer applications and analytical tools.
- Demonstrated leadership abilities.
Preferred Qualifications
- Experience with Electronic Health Records (EHR), particularly EPIC.
- ACM - Accredited Case Manager - American Case Management Association (to be completed within 3 years), or
- CCM - Certified Case Manager - Commission for Case Manager Certification (to be completed within 3 years).
Physical Demands
The role requires the ability to lift and carry up to 25 lbs, frequent sitting and standing, consistent keyboard usage, and may involve specific tasks such as kneeling, bending, squatting, and performing CPR as needed in patient care delivery.
