Family Health Matters Community Health Center is seeking a Medical Assistant II to provide advanced clinical and care coordination support within our Homeless Services Program. This role supports medical providers, behavioral health clinicians, and care teams in delivering trauma-informed, team-based care to vulnerable populations.
This position operates in a fast-paced, high-acuity environment and plays a key role in supporting continuity of care, outreach, and population health efforts.

What You’ll Do
Clinical Support
- Prepare patients for visits, including intake, medication reconciliation, and history updates
- Perform vital signs and point-of-care testing (A1C, glucose, UA, pregnancy tests)
- Assist with wound care, immunizations, injections, and minor procedures
- Support Medication Assisted Treatment (MAT) services
- Assist providers during exams and procedures
Homeless Services Support
- Provide care using trauma-informed and harm-reduction approaches
- Assist with services in shelter-based or mobile clinic settings
- Coordinate care with shelter staff and case managers
- Recognize and respond appropriately to behavioral health concerns
- Support care continuity for high-risk patients
Care Coordination & Team-Based Care
- Support outreach efforts and follow-up with patients
- Assist with referrals and warm hand-offs
- Participate in daily team huddles and case conferences
- Collaborate closely with:
- Medical providers
- Behavioral health teams
- Dental and optometry teams
- ECM and community health teams
Compliance & Documentation
- Maintain accurate EHR documentation
- Support PPS billing documentation requirements
- Follow infection control, OSHA, HIPAA, and safety standards
- Maintain required certifications and competencies
What We’re Looking ForRequired
- Graduate of an accredited Medical Assistant program
- Current Medical Assistant Certification (CMA, RMA, or equivalent)
- Current BLS Certification
- Minimum 2 years of clinical experience
- Ability to work in mobile or community-based settings
- Valid California driver’s license (if mobile assignments required)
Preferred
- Experience working with vulnerable or homeless populations
- Bilingual Spanish/English strongly preferred
- Experience in community health or FQHC settings
- Experience supporting behavioral health or MAT services

Schedule: Full-time hours, AWW schedule 7:00am to 6pm; final schedule will be discussed during the interview.
Work Environment
This position may include work at clinic sites, shelters, or mobile service locations and supports individuals experiencing homelessness in a trauma-informed care environment.
