We are seeking a dedicated and heart-centered Social Worker to join our interdisciplinary team serving older adults and adults with disabilities in a warm, supportive Adult Day Health Care setting.

Job Summary:
- Conduct psychosocial assessments and individualized care planning
- Provide emotional support, counseling, and crisis intervention
- Support participants experiencing isolation, depression, anxiety, grief, or adjustment challenges
- Assist families and caregivers with education, resources, and long-term planning
- Coordinate with healthcare providers, community agencies, and support services
- Participate in interdisciplinary team meetings and participant care conferences
- Maintain accurate documentation in compliance with California ADHC regulations
- Advocate for participant rights, dignity, safety, and quality of care
Qualifications:
- Master's degree in Social Work (MSW) or other related field
- or Bachelors degree in Social Work or other related field and 2 years of experience in a healthcare or social services setting
- Strong communication and interpersonal skills with a compassionate demeanor.
- Ability to work collaboratively in a multidisciplinary team environment.
- Knowledge of community resources and services for seniors.
- Proficient in documentation and familiarity with electronic health records.
- Commitment to ongoing professional development and ethical practice.
Additional Requirements:
Current CPR and First Aid certification
Bilingual skills are a plus (Vietnamese, Korean)
Position Status:
Full-time position (32-35 hours per week)
