Home Health Registered Nurse (RN) | Philadelphia, Pennsylvania

An opportunity is available for an experienced and compassionate Registered Nurse skilled in home health care. This contract role involves providing high-quality, patient-focused nursing services directly in patients homes. The position requires clinical expertise, autonomy, and a dedication to patient and family education, making a real impact on the continuum of care.

Schedule & Details:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points weekly (approximately 35 patient visits per day)
• Field-based, providing care within assigned territories
• Utilizes Home Care Home Base (HCHB) for electronic documentation and OASIS assessment
• Daily travel with mileage reimbursement

Core Responsibilities:

• Complete comprehensive OASIS assessments, develop, and manage patient-centered care plans
• Deliver skilled nursing care: wound treatment, IV therapy, medication management, and chronic condition oversight
• Monitor patient progress and promptly communicate with the interdisciplinary care team and physicians
• Provide clear education to patients and families about treatments, medication regimens, and disease processes
• Maintain timely, accurate documentation within HCHB
• Collaborate with physical/occupational therapists, medical social workers, and home health aides to ensure continuity of care
• Attend case conferences, maintain educational competency, and comply with all regulatory and agency protocols

Required Qualifications:

• Active RN license in good standing
• Minimum one year of recent home health (field-based) RN experience
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Strong assessment, organizational, and communication skills
• Reliable vehicle, valid drivers license, and proof of auto insurance
• Comfort working independently and managing a flexible field-based schedule

Preferred:

• BSN degree
• Experience handling a caseload of 2532 points per week
• Understanding of Medicare and home health compliance
