We are currently seeking a motivated and compassionate Physical Therapist Assistant (PTA) to join our growing team of healthcare professionals.
Position Summary
As a Home Health Physical Therapist Assistant, you will work under the supervision of a licensed Physical Therapist to provide skilled therapy treatments in patients' homes. This role offers the opportunity to build meaningful relationships with patients while helping them regain mobility, improve strength, and increase overall independence.
The ideal candidate is dependable, organized, and passionate about delivering quality patient care in a home health setting.
Responsibilities
- Conduct comprehensive physical therapy evaluations in the home setting
- Develop individualized treatment plans based on patient needs and goals
- Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and functional independence
- Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety strategies
- Monitor patient progress and modify treatment plans as appropriate
- Complete accurate and timely clinical documentation
- Collaborate with physicians, nurses, case managers, and interdisciplinary team members to ensure continuity of care
- Maintain compliance with all state, federal, and agency regulations
Qualifications
Required
- Active California Physical Therapist Assistant (PTA) License
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR Certification
- Valid driver's license and reliable transportation
- Strong communication and interpersonal skills
- Ability to work independently while managing a flexible schedule
Preferred
- Home Health experience
- Experience working with geriatric patients
- Familiarity with electronic medical record (EMR) systems
