Role Overview
Our agency is currently seeking to engage a qualified Intake / Scheduling Coordinator to support our home health operations. We are looking for a highly efficient, detail-oriented professional with strong experience in end-to-end intake coordination and home health workflow management. This role is critical to ensuring timely patient admission, accurate documentation, and efficient clinician scheduling.
Key Responsibilities
Processing and entering referrals into Axxess EMR
Scheduling clinicians appropriately based on discipline, availability, and payer requirements
Obtaining and verifying home health insurance authorizations
Reviewing and processing physician orders
Faxing and following up on orders with physicians
Uploading and maintaining documentation and orders in Axxess
Managing intake workflow from referral through admission and scheduling
Understanding Medicare home health guidelines, including OASIS requirements and compliance expectations
Working efficiently in a fast-paced, high-volume environment with minimal supervision
Qualifications
Highly organized and detail-oriented
Experienced in home health intake or coordination roles
Able to work independently with strong follow-through
Comfortable managing multiple priorities simultaneously
Familiar with Medicare Conditions of Participation and home health compliance workflows
Strong in communication with clinicians, physicians, and internal staff
