The RN Home Visitor is a full-time, onsite role within the Nurse Family Partnership program, responsible for providing case management and support services to pregnant and parenting women and their children.
Responsibilities include conducting home visits, collaborating with community agencies, ensuring compliance with regulatory standards, and following program policies and protocols.
The position requires demonstrating professionalism, promoting quality care across physical, psychological, social, educational, and safety needs, and maintaining high standards in clinical practice and teamwork.
Qualifications include a valid RN license, BLS certification, a BSN within five years, and at least two years of recent pediatrics or related experience. Experience with adolescents, OB, home care, or community nursing is preferred. A valid driver’s license and a safe, insured vehicle are required.
Other duties may be assigned as needed.
