LPN Primary Care Manager: Behavioral Health Home- 6533

OBJECTIVE OF POSITION:

To provide high quality integrated medical and behavioral health care coordination, services and oversight of CHR’s Behavioral Health Home enrollees to improve health outcomes.

By:

• Providing high quality integrated medical and behavioral health care coordination, services and oversight of CHR’s Behavioral Health Home enrollees to improve health outcomes of adults and/or children in CHR’s Behavioral Health Homes (BHH).
• Assisting in developing and coordination of health and wellness initiatives for adult and/or child BHH identified patients.
• Working closely with BHH Behavioral Health Home and other agency staff to achieve high quality, well-coordinated medical and behavioral health care and improved health outcomes for CHR’s BHH enrollees.
