Bring your clinical expertise and patient-centered approach to a dynamic field-based nursing position. As a Home Health Licensed Practical Nurse (LPN), you will deliver skilled care that promotes independence and optimal outcomes for patients in their homes. This rewarding contract assignment allows you to support individuals during recovery, focusing on education, compliance, and comprehensive care planning.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM (with occasional schedule flexibility for patient needs)
Key qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum of one years experience in home health, hospice, or a similar community-based care role
- Proficiency in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable transportation, and current auto insurance
- Current BLS (Basic Life Support) certification
- Strong clinical judgment, effective communication skills, and the ability to manage a diverse caseload independently
Preferred background:
- Experience with Medicare-certified home health agencies
- In-depth understanding of home health compliance and documentation standards
- Excellent interpersonal and patient teaching skills
Primary responsibilities:
- Conduct skilled nursing visits to assess, implement, and revise individualized care plans
- Administer medications, perform wound care, and manage clinical interventions following physician orders
- Monitor and document patient status and promptly communicate changes to the RN Case Manager or other providers
- Educate patients and families on disease management, medications, and self-care
- Maintain compliance with infection control, regulatory, and organizational standards
- Participate in interdisciplinary case conferences, ongoing training, and quality improvement activities
