Director of Case Management

Humboldt Park Health is seeking a Director of Case Management! This leadership position is providing oversight and is responsible for all aspects of organizational evaluation, planning and management. The Director of Case Management is also responsible for ongoing staff development and patient quality / safety. Additional duties that this role entails include product line development, fiscal oversight and departmental positioning within the community!
