As an Occupational Therapist at Sunrise Health Services, you will play a vital role in helping patients regain independence and improve their quality of life through individualized treatment plans and therapeutic interventions. You will assess patients in their home environment, develop personalized care plans, recommend adaptive equipment and home modifications, and collaborate closely with physicians, nurses, and the interdisciplinary care team to ensure optimal patient outcomes. This position offers flexibility in scheduling and work hours, making it an excellent opportunity for both full-time and part-time candidates who are passionate about delivering compassionate, patient-centered care in the comfort of patients' homes.
Key Deliverables:
- Assists the physician in evaluating the patient's level of function.
- Assess patient's home environment for safety hazards and architectural barriers identify equipment needs.
- Identifies functional problems and establishes goals and a plan of care designed to improve the patient's functional status (revising as necessary).
- Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
- Performs and completes start-of-care, and ongoing assessments and associated paperwork in accordance with agency policy.
- Periodically re-evaluates and upgrades the patient's program to maximize functional improvement.
- Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
- Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
- Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthetist/orthotist.
- Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
- Performs and submits required documentation of patient's evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
- All patient visit notes must be turned in the next scheduled workday, not to exceed forty-eight (48) hours.

- Notifies patient/clients prior to the visit and communicates further visits scheduled.
- Drives responsibly and safely to and from the client's home.
- Schedules own time-based on priorities and maintain productivity utilizing time management.
- Provides appropriate and thorough service to patients/clients, ensuring proper completion of services in an accurate and timely manner and in accordance with agency policy.
- Makes the initial patient/client Start of Care (SOC) assessment within appropriate time as scheduled by
the agency.
- Always demonstrates respectable customer service and professionalism.
