Per Diem Nurse Practitioner Home Health | Jacksonville Beach, Florida

Provide critical care and support to patients in a home health setting as a Nurse Practitioner conducting risk assessments throughout Duval County, FLincluding Jacksonville Beach, Jacksonville, and Atlantic Beach. This per diem opportunity offers flexibility and autonomy, as practitioners can create their own schedules with shifts available any day of the week.

Schedule:

• Flexible shifts between Monday and Sunday
• Option to create your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct in-depth annual home health risk assessments for Medicare Advantage health plan members
• Review and evaluate current and historical medical information, including conditions and medication usage
• Deliver patient education on medical conditions and medications
• Identify potential gaps in care and offer actionable recommendations for ongoing management
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Florida
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• Minimum of six months of recent Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
