Physical Therapist - In-Home & Senior Living Care

• **Key Responsibilities:***
• Deliver individualized physical therapy to older adults.
• Conduct evaluations and develop tailored care plans based on the latest evidence.
• Efficiently document patient care within our electronic medical record (EMR) system while adhering to regulatory standards.
• Collaborate with a multidisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, their caregivers, and senior living staff to enhance safety and promote functional independence.
• Provide telehealth/virtual care options to improve access for our clients.

• PRN / FOX Flex Physical Therapist - Home & Community Visits (pay per unit compensation)

• **Physical Requirements:***
• Must be able to handle the physical demands of in-home and community care, including:
• Medium physical effort, occasionally lifting 25-50 lbs
• Frequent lifting of 10-25 lbs
• Constant lifting up to 10 lbs
• Ability to perform CPR and provide assistance with patient transfers and mobility.
• Willingness to drive to patient homes and senior living communities.
• Availability to wear PPE when required.
