Responsibilities

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor and observe patient conditions, accurately documenting vital signs and health status in electronic health record (EHR) systems
  • Support medication administration and ensure adherence to prescribed treatment plans under supervision
  • Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques
  • Perform catheter care, wound care, and assist with hospice or end-of-life care as needed
  • Help with light housekeeping duties such as cleaning, laundry, meal preparation, and maintaining a safe environment
  • Follow home healthcare regulations and HIPAA guidelines to protect patient privacy and confidentiality

Qualifications

  • Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred
  • Knowledge of medical terminology and medical documentation practices
  • Certification in CPR (Cardiopulmonary Resuscitation) and Basic Life Support (BLS) required; additional certifications such as First Aid are a plus
  • Ability to lift heavy objects safely using equipment like Hoyer lifts when necessary
  • Experience working with geriatric patients or individuals with disabilities or developmental disabilities highly desirable
  • Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation
  • Strong communication skills to effectively interact with patients, families, and healthcare teams
  • Valid driver’s license and reliable transportation preferred for travel between patient homes
