Position Overview

We’re seeking a Home Health Registered Nurse to deliver skilled nursing care in patients’ homes within a 60‑mile radius of Peoria, AZ. The role includes a 36‑hour weekly guarantee, a Monday–Friday 8:00 AM–4:30 PM schedule, and mileage reimbursement from the first patient to the last. An on‑call rotation is required.

This Job Will Have The Following Responsibilities

• Skilled Nursing Care — Provide comprehensive nursing care in the home setting, including assessments, wound care, medication management, patient education, and coordination with interdisciplinary teams.
• Patient and Family Education — Teach patients and caregivers about disease processes, medications, safety, and self‑management strategies.
• Interdisciplinary Collaboration — Communicate effectively with physicians, therapists, and case managers to ensure continuity of care.
• Clinical Documentation — Maintain accurate, timely documentation aligned with agency standards and regulatory requirements.
• Care Planning — Develop, implement, and update individualized care plans based on patient needs and clinical findings.

Qualifications & Requirements:

• Registered Nurse (RN) license in the state of Arizona
• Strong clinical assessment and critical‑thinking skills
• Ability to work independently in a field‑based role
• Home Health Experience
