Pride Health is currently seeking candidates for an "LPN"_ position that will provide support to our client's healthcare systems in Dallas, Texas. This is a contract assignment _with a high likelihood of extension and a great way to start working with a top-tier healthcare organization!
Job Description:
Job Title: LPN – Home Health
Location: Dallas, TX
Schedule: 40 hours/Week
Days: Days, 8X5
Pay Range for Traveler: $30 to $30 per hour
Responsibilities:
- Provide skilled nursing care to patients in their homes under the supervision of an RN or physician.
- Perform patient assessments and monitor vital signs.
- Administer prescribed medications, injections, and treatments.
- Perform wound care and dressing changes.
- Monitor and report changes in the patient's condition to the RN or physician.
- Assist with catheter care, ostomy care, and tube feeding as needed.
- Educate patients and family members on medications, disease management, and home care.
- Maintain accurate and timely documentation in the electronic medical record (EMR), often using Homecare Homebase (HCHB).
Required Qualifications:
- Minimum of 1 years of experience in a hospital, skilled nursing facility, or comparable setting
- Current CPR/BLS certification required.
