Role Description
The Hospice RN Case Manager is a full-time, on-site position serving patients throughout the San Francisco Bay Area. The Hospice RN Case Manager is responsible for coordinating and managing all aspects of care for an assigned caseload of hospice patients in accordance with physician orders, the interdisciplinary plan of care, Medicare Conditions of Participation, and California hospice regulations.
Responsibilities include conducting comprehensive patient assessments, developing and updating individualized plans of care, managing pain and symptoms, providing skilled nursing interventions, and coordinating services with physicians, social workers, chaplains, hospice aides, therapists, and other members of the interdisciplinary team. The RN Case Manager serves as the primary clinical coordinator for assigned patients, ensuring continuity of care and timely communication among all disciplines.
Additional duties include patient and family education regarding disease progression, medications, comfort measures, and hospice services; supervision of LVNs and Hospice Aides in accordance with California regulations; participation in interdisciplinary group meetings; accurate and timely clinical documentation; on-call responsibilities as assigned; and compliance with all agency, state, federal, and accreditation requirements.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the State of California.
- Graduate of an accredited school of nursing; BSN preferred.
- Minimum one (1) year of recent nursing experience required.
- Hospice, palliative care, home health, oncology, geriatrics, or medical-surgical nursing experience preferred.
- Demonstrated knowledge of hospice philosophy, end-of-life care, pain and symptom management, and interdisciplinary care planning.
- Ability to perform comprehensive physical, psychosocial, emotional, and environmental assessments.
- Strong case management, care coordination, and clinical decision-making skills.
- Excellent communication and interpersonal skills when working with patients, families, physicians, and interdisciplinary team members.
- Experience with electronic medical records and hospice documentation requirements.
- Current BLS certification required.
- Valid California driver's license, reliable transportation, and current automobile insurance.
- Ability to travel throughout assigned service areas.
- Ability to supervise Hospice Aides and LVNs in accordance with California hospice regulations.
- Knowledge of Medicare Hospice Conditions of Participation and California hospice licensing requirements preferred.
- CHPN certification is a plus.
- Ability to participate in weekend, holiday, and on-call rotations as required.
